Please be advised that any procedures or additional treatment provided to your child other than the intended doctor visits are billable.
- $50 No show without 24 hour notice of cancellation
- Forms: 1st form filled out at a well visit; no cost
- Each additional form at visit: $20
- Forms dropped off/Urgent forms: $20
- Sports Physical: $55 (Needs appointment with doctor)
- Drivers License Physical: $55 (Needs appointment with doctor)
